RESUMO

In view of the problems existing in the probation teaching of obstetrics and gynecology, such as patients' non-cooperation, few cases and few practical opportunities. This study discusses the application effect of case teaching based on "Rain Classroom" and standardized patients in the obstetrics and gynecology probation course. The teaching is carried out by carefully selecting cases, refining pre-class preparation, adopting various teaching forms, and carrying out evaluation after class. In order to evaluate the teaching effect, an anonymous survey was carried out among 330 undergraduate interns majoring in clinical medicine, prevention, and forensic medicine in Batch 2015. The results showed that 210 (93.8%) of 224 (67.9%) students who participated in the survey believed that this method could promote learning enthusiasm and 200 students (89.3%) thought it could activate classroom atmosphere more than traditional teaching; most students thought it improved students' communication ability (163, 72.8%), clinical practice ability (189, 84.4%), and ability to analyze and solve problems (171, 76.3%) and deepening the understanding, mastery and application of knowledge (158, 70.5%). It shows that this method can improve the teaching effect of obstetrics and gynecology probation course to a certain extent.

Objective To explore the prevalence and epidemiological characteristics of bronchial asthma in people over 40 years old in Hai'an. Methods The stratified random sampling method was used in this study. A questionnaire survey was conducted and pulmonary function measurement was performed among the residents over 40 years old in Hai'an, ​Jiangsu Province to analyze the prevalence and epidemiological characteristics of asthma. Results A total of 2 237 valid questionnaires were collected in the survey, and 116 patients with bronchial asthma were found, with an incidence rate of 5.19%. The prevalence of asthma was related to gender, age, family history, active and passive smoking, occupational exposure, childhood history of lung disease, history of respiratory allergies, and exposure to cooking fumes (P<0.05). Multivariate logistic regression analysis showed that being male (OR=0.637, 95%CI: 0.499~0.691) was a protective factor for asthma, while advanced age (OR=1.438, 95%CI: 1.207~1.972), active smoking (OR=1.795, 95%CI: 1.423~2.399), passive smoking (OR=2.098, 95%CI: 1.423~2.399), occupational exposure (OR=3.446, 95% CI: 2.175~6.290), childhood history of lung disease (OR=2.228, 95%CI: 1.337~4.491), history of respiratory allergies (OR=2.287, 95%CI: 1.535~5.219) were risk factors for asthma (P<0.05). Conclusion The prevalence rate of bronchial asthma in people over 40 years old in Hai'an was high. Advanced age, active and passive smoking, occupational exposure, a history of lung disease in childhood, and a history of respiratory allergies were related to asthma development. It is necessary to strengthen the prevention and treatment of asthma among high-risk population in clinical practice to reduce the incidence of asthma.

Objective To discuss the expression and significance of neutrophil to lymphocyte ratio(NLR) and platelet to lymphocyte ratio(PLR) in peripheral blood in patients with chronic obstructive pulmonary disease(COPD).Methods One hundred and fifty cases COPD patients who were treated in Respiratory Departments of Hai'an People′s Hospital Affiliated to Nantong University from January to December 2015 were selected and divided into stable period group with 60 cases and acute exacerbation period group with 90 cases,and 50 healthy subjects were selected as control group.Clinical data were collected and white blood cell count(WBC),neutrophil count(NC),lymphocyte count(LC),platelet(PLT),NLR,PLR,forced expiratory volume in one second(FEV1%) predicted and C reactive protein(CRP) were tested.Results NLR,PLR and CRP values were significantly higher in patients in acute exacerbation period group than in stable period group and control group(NLR:(10.30±9.93) vs.(6.76±5.90) vs.(5.85±3.67);PLR:(335.88±164.94) vs.(286.80±118.98) vs.(194.11±88.14);CRP: (42.18±24.89) mg/L vs.(23.27±13.59) mg/L vs.(22.49±13.47) mg/L,F=6.637,6.808,11.007,P0.05).There was a significant positive correlation between NLR,PLR and CRP in acute exacerbation period group(r=0.374,0.379,P<0.01),there was a positive correlation between NLR and PLR(r=0.482,P<0.01),there was a negative correlation between NLR,PLR and FEV1(r=-0.297,-0.299,P<0.05).There was a significant positive correlation between NLR,PLR and CRP in table period group(r=0.593,0.433,P<0.01),NLR and PLR were also positively correlated(r=0.618,P<0.01),NLR,PLR and FEV1 accounted for the expected value of the negative correlation(r=-0.324,-0.342,P<0.05).Conclusion Both NLR and PLR are inflammatory markers,furthermore they show a significant negative correlation to airflow limitation in patients with COPD.

Objective To explore the role of hypoxia-inducible factor 1α(HIF-1α)and carbonic anhydrase IX (CAIX)in oncogenesis and development of gestational trophoblastic disease (GTD).Methods The expressions of HIF-αand CAIX were determined immunohistochemically by specific antibodies on formalin-fixed paraffin sections of normal pregnancy villi (32 cases),hydatidiform moles (33 cases),and invasive hydatidiform moles (26 cases). Results The positive rate of HIF-1α in normal pregnancy villi (NP),hydatidiform moles (HM)and invasive hydatidiform moles (IM) was 28.1% (9/32 ), 45.5% (1 5/33 ) and 73.1% (1 9/26 ), respectively. It was significantly higher in IM than in HM and NP(P 0.05).The positive rate of CAIX in NP,HM and IM was 31.2 (10/32),3.0(1/33)and 57.7%(1 5/26),respectively.It was significantly higher in IM than in HM and NP (P 0.05).Conclusion The expressions of HIF-1αand CAIX were both higher in gestational trophoblastic neoplasia than in normal pregnancy and hydatidiform moles, which indicates that HIF-1αand CAIX may play a role in oncogenesis and development of malignant transformation of trophoblastic cells.

<p><b>BACKGROUND</b>Prolactin (PRL) is a pituitary polypeptide hormone characterized by multiple biological actions including stimulation of growth in the prostate and formation of secretory alveoli and stimulation of milk protein gene expression in the mammary gland. PRL exerts its effect by dimerizing its receptor (PRLR) on the plasma membrane and regulating gene expression through the JAK-Stat signal pathway. We have previously described a natural variant of the PRLR in which the S2 subdomain of the extracellular domain is missing (Delta S2). Delta S2 PRLRs are dimerized in the absence of PRL and have constitutive activity in the promotion of breast cancer cell growth. Enhancer of zeste homolog 2 (EZH2), as one of the histone-modifying enzymes, is a key factor regulating gene expression by epigenetic modification. We hypothesized that these constitutive activated Delta S2 PRLRs played a pathogenic role in breast cancer in part through alterations in the expression of EZH2 and the trimethylation of histone 3 on lysine 27 (H3K27Me3).</p><p><b>METHODS</b>In order to verify the clinical significance and to establish the link between Delta S2 PRLR expression and epigenetic change, EZH2, H3K27Me3, and Delta S2 PRLR were detected in both normal and cancerous human breast tissues. Also, overexpression of Delta S2 PRLR in breast epithelial cells was achieved by infection with adenovirus carrying the cDNA. Western blotting and chromatin immunoprecipitation (ChIP assay) and acid histone extraction were applied to detect the expression of EZH2 and the trimethylation of histone 3, respectively.</p><p><b>RESULTS</b>In breast tissue, higher EZH2 expression and higher H3K27Me3 were found associated with higher Delta S2 expression in breast cancer samples. In breast epithelial cells, overexpression of Delta S2 PRLR increased EZH2 methyltransferase mRNA and protein, induced EZH2 methyltransferase recruitment to chromatin, increased the trimethylation of H3K27Me3, and decreased the expression of p53 gene.</p><p><b>CONCLUSIONS</b>Delta S2 PRLR plays an important pathogenic role in breast cancer through epigenetic modification. Elevated expression of Delta S2 PRLR, achieved by alternate splicing of the pre-mRNA of the full-length form, is a new mechanism contributing to human breast cancer.</p>

OBJECTIVE: To optimize the MR imaging protocol for coronary arterial wall depiction in vitro and characterize the coronary atherosclerotic plaques. MATERIALS AND METHODS: MRI examination was prospectively performed in ten porcine hearts in order to optimize the MR imaging protocol. Various surface coils were used for coronary arterial wall imaging with the same parameters. Then, the image parameters were further optimized for high-resolution coronary wall imaging. The signal-noise ratio (SNR) and contrast-noise ratio (CNR) of images were measured. Finally, 8 human cadaver hearts with coronary atherosclerotic plaques were prospectively performed with MRI examination using optimized protocol in order to characterize the coronary atherosclerotic plaques. RESULTS: The SNR and CNR of MR image with temporomandibular coil were the highest of various surface coils. High-resolution and high SNR and CNR for ex vivo coronary artery wall depiction can be achieved using temporomandibular coil with 512 x 512 in matrix. Compared with histopathology, the sensitivity and specificity of MRI for identifying advanced plaques were: type IV-V (lipid, necrosis, fibrosis), 94% and 95%; type VI (hemorrhage), 100% and 98%; type VII (calcification), 91% and 100%; and type VIII (fibrosis without lipid core), 100% and 98%, respectively. CONCLUSION: Temporomandibular coil appears to be dramatically superior to eight-channel head coil and knee coil for ex vivo coronary artery wall imaging, providing higher spatial resolution and improved the SNR. Ex vivo high-resolution MRI has capability to distinguish human coronary atherosclerotic plaque compositions and accurately classify advanced plaques.

Objective To get a MR imaging protocol for coronary arterial wall in vitro. Methods MR examinations were performed in 10 fresh porcine hearts. Three dimensional fast imaging employing steady state acquisition (3D FIESTA) was used to delineate left anterior descending artery (LAD), while 2D spin-echo T1W was performed with 8-channel head surface coil, temporomandibular surface coil and knee coil with the same parameters. T1WI was obtained with 384×256 and 512×512 in matrix using temporomandibular surface coil, and then T1WI, PDW and T2WI with fat saturation were obtained with different NEX using temporomandibular surface coil after injecting Resovist in LAD. Signal of the LAD wall, lumen, fat tissue adjacent to LAD, myocardium of anterior part of interventricular septum and noise were respectively measured. Signal-to-noise ratio (SNR) of image, contrast to noise ratio (CNR) between the wall and lumen (CNR1), CNR between the wall and surrounding fatty tissue (CNR2) were calculated. Results The SNR and CNR1, CNR2 of SE T1WI with temporomandibular coil were higher than those with 8-channel head surface coil and knee coil. SNR and CNR1, CNR2 of SE T1WI with 384×256 matrix were higher than those with 512×512 matrix. SNR and CNR1, CNR2 using 3 NEX were the highest. Conclusion Good SNR and CNR of porcine coronary wall can be achieved using temporomandibular surface coil, 384×256 in matrix and NEX of 3.

RESUMO

This study was conducted to prepare and determine the content and entrapment efficiency of Clindamycin Phosphate liposome. Evaporating and Ultrasound method was used for preparing Clindamycin Phosphate liposome. HPLC was used for determining the concentration and the entrapment efficiency of Clindamycin Phosphate liposome. The results indicated that Clindamycin Phosphate had a good linear relation in a range of 5.0-50.0 microg/ml. The entrapment efficiency of Clindamycin Phosphate liposome in three groups reached 52.26%, 50.13%, 53.75% respectively. Accordingly, the technique of preparing Clindamycin Phosphate liposome was noted to be feasible, and the method of quality control was shown simple and accurate.

Objective To observe PDX-1 expression in the pancreas of type 2 diabetic rat model and effects of rosiglitazone (RGZ) on it. Methods Type 2 diabetic rat model was induced by long-term feeding with high-fat foods followed by a single intraperitoneal injection of low dose of streptozotocin (STZ). The morphological changes of pancreas were examined by microscopy and immunohistochemistry. The mRNA levels of PDX-1 and insulin were determined by RT-PCR. PDX-1 protein expression was detected with Western blot. Results The percentage of insulin-positive cells(12.75±2.18),the average optical densities of PDX-1-positive area(0.240±0.051),PDX-1 mRNA level (0.153±0.071)and PDX-1 protein level(0.253±0.028) were significantly lower in untreated diabetic rats than in the controls(42.61±2.68,0.648±0.087,0.49±0.032,0.720±0.036 respectively)(all P<0.01),and in RGZ-treated versus untreated group those parameters increased significantly (all P<0.05~0.01). Conclusions Rosiglitazone could protect islet cell function,and improve PDX-1 expressions in both mRNA and protein levels.

Objective To study the genome DNA methylation in SLE and the related factors of DNA methylation. Methods Twenty-six cases with SLE and 20 controls were recruited to participate the study. Plasma Hcy, SAM, SAH and the MTHFR gene C677T polymorphism were measured in all patients and controls. Results {1} The SAM levels were lower significantly in SLE groups than in controls.
